Become a Provider

What certifications do I need to be a provider?

  • First Aid/CPR

  • Fingerprint Clearance Card

  • Article 9 Training

  • DCW (if providing ATC)

Where can I get training for this position?

  • First Aid/CPR is available online as well as in person through HeartSavers, National CPR Foundation, as well as others.
     

  • Fingerprint Clearance Cards can be obtained by submitting a form to the Department Of Public Safety

  • Article 9 Training is done through Arizona Provider Training 

 

  • DCW or Direct Care Work (if providing attendant care) is also done through Arizona Provider Training
